Oltre il crinale che divide l'Emilia dalla Toscana, la Strada Ducale attraversa il piccolo nucleo di San Pellegrino, antico ospizio per viaggiatori e sede del Santuario omonimo, che la tradizione popolare, non suffragata da alcun documento e contraddetta da più studiosi, vuole fondata dal Re di Scozia. Il primo documento nel quale si citano la Chiesa, le case ed il "Spetale" di San Pellegrino risale al 1110. Nel 1449 il Rettore Leonello De Nobili  (Abate di Frassinoro), ristruttura il complesso riducendo la Chiesa ed altri fabbricati. Il tempietto di Matteo Civitali che contiene le spoglie dei Santi San Pellegrino e San Bianco è di poco successivo (1478 - 1479).

Beyond the crest which divides Emilia from Tuscany, the Strada Ducale ('Ducal Road') crosses the little village of San Pellegrino. Traditionally a stopping point for travellers, the village has a sanctuary bearing the same name. According to popular belief, this was founded by the King of Scotland, but there are no documents to back this up. The church, the houses and the 'Spetale' of San Pellegrino are first mentioned in a document which dates back to 1110. In 1449 the rector, Leonello De Nobili (the abbot of Frassinoro) restored the complex and reduced the church and the other buildings. The small temple, built by Matteo Civitali, where the remains of San Pellegrino and San Bianco are kept, was built not long after (1478/1479).
